Job Description

Position Summary: 

The Registered Nurse is responsible for the supervision and evaluation of assigned caregivers providing personal care services for the medically frail and medically compromised. Responsibilities include developing a service plan, updates, home supervisory visits, and interfacing with caregivers to ensure service plan is followed. Assisting with ongoing maintenance of client files and with ongoing administrative tasks as assigned.

Employee's Duties & Responsibilities:

  • Perform an initial comprehensive physical assessment of a client
  • Supervise caregivers providing personal care tasks to medically frail or medically compromised clients
  • Evaluate each assigned caregiver annually, either through direct observation or demonstration on the job tasks the caregiver is required to perform
  • No supervisor shall knowingly permit an employee who has been exposed to tuberculosis or hepatitis, or diagnosed with the same, to provide services to clients until it is determined
  • Complete a Client Service Plan for each assigned client
  • Make supervisory home visit to each assigned client's residence at least every 92 days, starting from the date of initial service, as the level of care requires, or if Synergy HomeCare of Calhoun received a complaint, ensuring that the client's needs are met if the caregiver is providing personal care task services. The visit shall include an assessment of the client's general condition, a review of progress being made, the problems encountered by the client, and the client's satisfaction with the services being delivered by Synergy HomeCare of Calhoun staff. Such supervision shall also include observations about the appropriateness of the level of services being offered. Routine quarterly supervisory visits shall be made in the client's residence and shall be documented in the client's file or service plan
  • If making a home visit when a complaint is received, document and follow Complaint and/or Incident Reporting policies and procedures
  • Be available to consult with on-call personnel as needed
  • Treat each caregiver and client with respect and dignity
Desired/Required Skills

Registered Nurse/LPN - qualified by training, education, and experience to supervise the provision of such tasks in accordance with accepted standards of care

CPR & First Aid certified

Annual negative TB test

Flexibility

Strong verbal and written communication skills
