Job Description: 

Under the general supervision of the Director of Clinical Services, Quality and Risk Management promotes patient care coordination for MedLink Health Center(s) by initiating referrals, scheduling appointments, communicating with insurance companies, answering patient calls and inquires and facilitating appropriate communication between providers, specialty providers, and patients.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES (include, but are not limited to, the following):

· Manages referral inbox, follow up box and closes the referral loop for HIV and HCV linkage to care.

· Assists with coordination of care by initiating referral orders sending requested documents, orders, or records for patient to requesting provider

· Schedules appointments for patients and/or sends documentation for scheduling as necessary.

· Reviews appointment information; date, time, location and provider name with patient

· Answers all telephone calls promptly in a polite and professional manner

· Answers questions and offers other information, as requested, to provide patient centered service and a positive impression of our organization

· Completes insurance verifications or prior authorizations as necessary to assist patient with referral process.

· Documents complete information in referral log for follow-up tracking.

· Routinely follows-up with patients and/or referral source to confirm patient has completed appointment and ensure consult notes have been received from referral source. Notifies supervisor of any difficulties in obtaining consult notes or results.

· Communicates with provider any gaps in patient care including follow-up, missed appointments, declined appointments, etc.

· Maintains accurate and confidential patient records following HIPPA guidelines and regulations

· Works with Chronic Care Management staff to refer patients as needed

· Participate in team and staff meetings as required or requested

· Other duties as assigned

Education/Experience: 

· Medical Assistant Certification with at least one year of experience/exposure to medical records. In lieu of a degree or certification, at least 2 years of referral, insurance verification, or medical office work experience.

· Strong verbal and written communication skills

· Strong computer skills with Microsoft Excel experience preferred

· Extremely dedicated in providing outstanding customer/patient service

· Excellent organizational skills and attention to detail

Salary: {Salary:18} / Per Hour

Hours: Full Time
